1. Brokerage Disclosure

Bedrock Insurance Advisors, LLC ("Bedrock") is a licensed insurance brokerage firm. We are not an insurance company and do not underwrite, issue, or guarantee insurance policies. Our role is to act as an intermediary between you and insurance carriers, helping you identify, evaluate, and obtain appropriate insurance coverage for your business needs.

As your broker, we represent your interests in the insurance marketplace. We work with multiple carriers to obtain competitive quotes and recommend coverage options, but the ultimate decision to bind coverage rests with the insurance carrier based on its underwriting criteria. All insurance policies are issued by the respective insurance carriers, and all claims are paid directly by those carriers subject to the terms, conditions, and exclusions of the applicable policy.

3. Compensation Disclosure

Bedrock Insurance Advisors is compensated for its services through commissions paid by insurance carriers. When we place an insurance policy on your behalf, the carrier pays us a commission that is a percentage of the premium you pay for the policy. Commission rates vary by carrier, line of business, and policy type.

In addition to standard commissions, we may receive the following forms of compensation:

• Contingent Commissions: Some carriers offer supplemental or contingent commissions based on the overall profitability, volume, or growth of the business we place with them. These arrangements do not affect the premiums you pay.
• Broker Fees: In certain circumstances, we may charge a broker fee for services rendered. Any broker fees will be disclosed to you in writing prior to binding coverage and are separate from, and in addition to, insurance premiums.
• Policy Fees: Some carriers include policy issuance or administrative fees that are collected at the time of binding. These fees are set by the carrier, not by Bedrock.

You have the right to request, at any time, information about the compensation we receive in connection with your insurance placement. To make such a request, please contact your Bedrock account representative or email us at certificates@bedrock-insurance.com.

4. Carrier Relationships

Bedrock Insurance Advisors operates as an independent insurance brokerage. We are not a captive agent or exclusive representative of any single insurance company. We maintain relationships with multiple admitted and non-admitted (surplus lines) carriers, which allows us to shop the market and present you with a range of coverage options and competitive pricing.

While we strive to present options from multiple carriers, we do not represent every insurance company operating in New York or any other state. The carriers we work with may change over time based on market conditions, underwriting appetite, and other business factors.

Our carrier recommendations are based on a combination of coverage suitability, pricing, carrier financial strength, claims service reputation, and our professional experience. We are not obligated to present every available option in the marketplace, but we are committed to acting in your best interest within the scope of the markets we access.

5. Surplus Lines Disclosure

Certain insurance coverages may be placed with non-admitted (surplus lines) insurance carriers. Surplus lines carriers are not licensed by the New York State Department of Financial Services to transact insurance in New York but are eligible to write surplus lines business under New York Insurance Law Article 21.

Important information about surplus lines coverage:

• Surplus lines policies are not covered by the New York Property/Casualty Insurance Security Fund (guaranty fund) in the event of the carrier's insolvency
• Surplus lines placements may be subject to additional state surplus lines taxes and stamping fees, which will be disclosed as part of the premium quotation
• Bedrock will only place coverage with surplus lines carriers when the required coverage is unavailable from admitted carriers, consistent with the diligent search requirements under New York law
• We select surplus lines carriers based on their financial strength ratings, claims-paying ability, and reputation in the marketplace

If your coverage is placed with a surplus lines carrier, we will provide you with written notice identifying the carrier and the surplus lines nature of the placement prior to binding.

6. Errors and Omissions (E&O) Coverage

Bedrock Insurance Advisors maintains professional liability (Errors and Omissions) insurance coverage. This coverage is designed to protect our clients in the unlikely event that a professional error or omission by our firm results in a financial loss to you. Our E&O coverage is maintained at limits that meet or exceed industry standards for insurance brokerages of our size and scope.

Information about our E&O coverage, including the carrier and policy limits, is available upon written request.

8. Anti-Fraud Disclosure

Pursuant to New York Insurance Law § 403, any person who knowingly and with intent to defraud any insurance company or other person files an application for insurance or statement of claim containing any materially false information, or conceals for the purpose of misleading information concerning any fact material thereto, commits a fraudulent insurance act, which is a crime, and shall also be subject to a civil penalty not to exceed five thousand dollars and the stated value of the claim for each such violation.

It is your responsibility to ensure that the coverage described in your policy documents matches your expectations and needs. If you identify any discrepancies, contact your Bedrock account representative immediately.

Renewal Disclosure: We will make reasonable efforts to contact you prior to the expiration of your policies to discuss renewal options. However, it remains your responsibility to ensure continuous coverage. Bedrock Insurance Advisors shall not be liable for any lapse in coverage resulting from non-renewal or delayed communication.
